Welcome to Enriched Nutrition. These Terms and Conditions outline the rules and regulations for the use of my services. By accessing my services, including personalized nutrition counseling and wellness programs, you agree to these terms. Please review them carefully before using my services.

Enriched Nutrition provides comprehensive nutrition counseling tailored to your personal health goals. My services are designed to empower you with knowledge and practical strategies for better health.

Individual Sessions: These sessions are ideal for clients who have a fundamental understanding of nutrition and seek flexible support. These are tailored to help clients independently manage their nutrition with periodic expert guidance.

Bundled Sessions: Bundled sessions are suitable for clients seeking a mix of structure and flexibility. These packages offer sessions at a discounted rate, providing ongoing support over a set period, which can be crucial for maintaining accountability and achieving specific health objectives.

Comprehensive Programs: My comprehensive programs offer a structured and holistic approach to nutrition and wellness. These are extensive plans that include regular sessions and continuous support, designed to address complex health goals through a strategic and supportive framework.

Scheduling and Cancellations

Scheduling: All appointments are booked through my online scheduling system, Practice Better. If you don't see a time that works for you, feel free to reach out directly at madison@enrichedrd.com and I'll do my best to accommodate you.

Cancellation Policy: I require at least 24 hours notice for canceling or rescheduling an appointment to avoid a fee. This policy helps me manage my schedule and assist other clients efficiently.

No-Show and Late Cancellation Fee: Appointments canceled or rescheduled with less than 24 hours’ notice will incur a fee equal to 50% of the scheduled service cost. No-show appointments will incur a fee equal to 100% of the scheduled service cost. For clients enrolled in a package or program, the missed session will be forfeited in lieu of a fee.

Payments

Fee Structure: Fees for services are due at the time of service. My pricing structure is clearly outlined during the consultation process.

Accepted Payment Methods: I accept all major credit cards. I do not accept Health Savings Account (HSA) or Flexible Spending Account (FSA) cards at this time.

Service Delivery

Mode of Delivery: All sessions are conducted virtually via a HIPAA-compliant electronic health record system, Practice Better, or via phone call, depending on client preference. I currently do not offer in-person sessions.

Health Insurance

Insurance and Reimbursement: While I do not directly accept insurance, I can provide a superbill upon request for clients who wish to seek reimbursement from their insurance providers. It is the client's responsibility to verify their insurance benefits.

Client Responsibilities

Client Commitment: Success in achieving health goals greatly depends on your active participation and commitment. Clients are expected to adhere to the planned nutrition and lifestyle recommendations and engage actively in their health improvement processes.

Eligibility: Services are available to individuals 18 years of age and older.

Limitation of Liability

Health Outcomes: Enriched Nutrition shall not be liable for any health issues that may arise from clients' non-compliance with the nutrition plans or failure to provide accurate health information.

Privacy and Confidentiality

Protection of Personal Information: I am committed to protecting your privacy. Client information is securely stored and handled with the utmost confidentiality and is only used as necessary for providing personalized counseling.

Modifications to Terms and Conditions

Updates to Terms: I reserve the right to modify these terms and conditions at any time. Changes will be effective immediately upon posting on my website. Continued use of my services after any such changes shall constitute your consent to such changes.
